Community Survey
This survey explores attitudes, belief and experiences concerning end-of-life issues, including hospice knowledge, advance care planning knowledge and practices, preferred medical practices, and spiritual support.
Community Survey with Pain and Symptom Knowledge Assessment
This survey is similar to the Community Survey but questions regarding beliefs about pain and symptom management are addressed in greater depth.
Clinical Experience Study Tools
This study utilizes family after-death interviews, medical chart reviews and surveys of the deceased patients’ professional providers to investigate patient and family experiences from a number of perspectives.
Faith Community Surveys
There are two faith community surveys –– one for clergy and and one for faith community members. The faith community leader survey tool is designed to assess strengths, weaknesses, and preparation of clergy in the provision of end-of-life spiritual care. The faith community member survey tool is designed to assess members’ feeling about the provision of end-of-life and bereavement care given by their faith community.
Family Bereavement Interview
This tool is an interview schedule of questions designed to explore caregivers' perspectives on dying family members’ quality of life, and the impact of being a caregiver and experiencing the illness and loss of a loved one.
Pain as the Fifth Vital Sign Research Tools
This is a collection of survey tools each designed for physicians, nurses, therapists, pharmacists and EMTs to assess competence and satisfaction with pain management practices.
