Task Forces: Advance Care Planning

The Representatives

Individuals from clinical settings in the community, as well as attorneys, chaplains, disability advocates, social workers, faith community leaders, and retirees.

The Focus

To foster a community in which people will have their health care choices honored at life's end.

The Results

  • My Choices (PDF), an advance directive form for use with adults of all ages, which contains a power of attorney for health care and a living will. To limit confusion and bring consistency to documenting people's preferences, task force members promoted the use of My Choices in legal, health care, spiritual care and other settings throughout the community; and members adopted its use in their own organizations.
  • The Choices Bank TM System,  a secure internet repository for advance directives.
  • The Wise Counsel Study – a survey distributed to attorneys and legal aides to assess advance care planning practices.
