Advance Care Planning and Choices Bank: Local
Life's End Institute and nine western Montana partners developed the Choices Bank TM, a secure web-based repository for health care advance directives, such as living wills and/or powers of attorney for healthcare.
Advance directives scanned into the Choices Bank TM system are password-protected and can be viewed through the Choices Bank website anytime, anywhere. The Choices Bank TM system provides peace of mind, knowing your health care choices will be available when they are needed.
